NOTE: If you complete the electronic PCS form with or without scheduling a transport online, please also print a copy of the PCS form and give it to Lifeguard’s crew when they arrive to transport the patient. If you choose “Schedule Transport and COMPLETE paper PCS form,” Lifeguard’s crew will need a completed PCS form when they arrive to transport the patient.
